That's super odd. Normally I would be pointing to the uninstall/reinstall procedure (with all the reboots and such) but honestly you strike me as having tried all the basics already.

Usually when this happens, it's been magically muted by something. Check out the 'Levels' tab in the control panel properties for the device, or eartrumpet is a nice pretty way to do it.
